Registration Instructions & Details 

Thank you for joining the Telluride Medical Center Foundation Classic! Here are a few notes to help your registration experience.

Foursome: Before you register your foursome, you will need name, email, handicap, and emergency contact for everyone in your group. If you are registering one foursome, you will choose the “4” quantity. This allows you to register 4 individual players during checkout. Don’t worry, you’ll only be charged for one foursome!

Foursome & Individual Registrations: Registration includes an invitation to the Calcutta, Breakfast, Golf, Lunch, & Awards.

Calcutta: Entry to the Calcutta is included in your foursome and individual registration. Additional tickets to the Calcutta can be purchased for your significant other and friends that aren’t attending the golf portion of the tournament.

Optional Add-Ons (available on the next screen): 

• Miracle Putts: $250 | Max of 2 miracle putts per team!
• Mulligans: $100 | Max of 2 mulligans per player!

Handicaps: Please accurately represent your handicap. Handicaps should reflect your current playing ability and be submitted accurately for tournament scoring and Calcutta purposes. If you don’t have a handicap, please enter “NH” for no handicap.
